Terms & Conditions

By engaging the services of the therapist, the patient acknowledges and agrees to these terms and conditions:

  1. Appointment Scheduling:
    • Appointment scheduling is done on a first-come, first-served basis.
    • We will confirm the appointment time and date with the patient prior to the visit.
    • If the client is unable to keep the scheduled appointment, a 24-hour cancellation notice is required.
  2. Fees:
    • Fees for physiotherapy services will be discussed and agreed upon before the first visit.
    • Payment is due at the time of service.
    • The patient will be responsible for any fees not covered by their insurance.
  3. Scope of Services:
    • The physio therapist will perform an evaluation and create a treatment plan based on the patient’s needs.
    • The therapist will provide treatment in accordance with the treatment plan.
    • The patient will be responsible for following the therapist’s recommendations for at-home exercises and self-care.
  4. Liability:
    • The physiotherapist will exercise reasonable care and skill while providing treatment.
    • The client understands that there are risks associated with physical therapy and accepts all risks associated with the treatment.
    • The therapist is not responsible for any injury or damage resulting from the patient’s failure to follow the therapist’s recommendations.
  5. Confidentiality:
    • The physical therapist will maintain the confidentiality of the patient’s health information in accordance with applicable laws and regulations.
    • The therapist will not disclose the client’s health information to any third party without the patient’s written consent, except as required by law.
  6. Termination of Services:
    • Either the patient or the physical therapist may terminate services at any time for any reason.
    • If the client terminates services, they must provide a 24-hour notice.
    • If the therapist terminates services, they will provide the client with a referral to another qualified provider.
  7. Dispute Resolution:
    • In the event of a dispute both the client and therapist agree to attempt to resolve the matter via mediation.
  8. Governing Law:
    • These terms and conditions shall be governed by and construed in accordance with the laws of the state of Queensland.
    • Any legal action arising out of or related to these terms and conditions shall be brought in the courts of the state of Queensland.
